Canby Camp Paperwork

CANBY CAMP is coming up fast and we want to make sure everyone is ready to participate. Below are the 3 things that need to be completed for participation in the camp. All forms are located in the DOCUMENTS Section of this APP, and online payment for camp is now available in the STORE section of the APP. Please completed these steps right away so we can have a great camp with everyone involved!!!

1) REGISTRATION FORM – The registration form can be found in the DOCUMENTS section of this APP. Please download it fill it out and mail it or send it electronically to Coach Boyer, ASAP.

2) CAMP PAYMENT – There is a $120 charge for this camp. This fee can be paid via the STORE section located in this APP or sent in with a check made out to Beaverton Football. If payment of this fee will keep you from attending, please email Coach Boyer at bob_boyer@beaverton.k12.or.us to discuss financial aide. We will not turn a player away just because they can not afford a camp!!!

3) PHYSICALS – The BSD Physical Form can be found also in the DOCUMENTS section of this APP. This will need to be completed fully by a physician and turned in prior to the start of actual practices. The BSD trainers will host a Physical Night at Mountainside HS on May 24th, 6-7:30, for $20. All proceeds go back to our trainers at the school!!! All in coming 9th graders, current Sophomores and anyone new to sports at BHS must complete a Physical prior to the 1st practice.

May 29/30 – HS MTGS 2:50 – 4:30 in Lecture Hall
May 31st – Gear Handout at BHS
June 1st – practice All Grades 4:30-7pm
June 2nd – All Grades 9-11:30
June 4-7th – All Grades 4:30-7pm
June 8th – All Grades 4:30-7:30pm at Liberty HS
June 9th – All grades 8-12noon at Liberty HS
